Requirements for New Patients:

  • Two (2) Government Issued IDs

  • Passport for Foreign Nationals
  • Original Marriage Certificate or Certificate of No Marriage (CENOMAR) for single patients

  • Previous Medical Records pertaining to reproductive or infertility work-up
